Broker Compensation Disclosure Form
In compliance with the Consolidated Appropriations Act, 2021 (CAA), entities providing brokerage and consulting services, including their affiliates and subcontractors, must disclose any expected direct or indirect compensation of $1,000 or more to plan fiduciaries. This disclosure must be provided in writing “reasonably in advance” of when the contract is entered into, extended, or renewed.
To facilitate this process, we offer a Broker Compensation Disclosure Form that meets CAA requirements. This form should be completed annually by your current broker or consultant to ensure transparency and compliance.
Feel free to use the form linked below to verify that you are receiving the necessary disclosures.
